Per Diem Compensatio n: visit rate + mileage + stipend Position Summary Aura Home Health & Hospice is seeking a dependable and compassionate Licensed Vocational Nurse (LVN) to provide overnight on-call coverage for patients in Santa Rosa and surrounding communities(Rohnert Park, Petaluma, Novato, Marin, etc,.) The Overnight On-Call LVN will serve as a clinical resource for patients, families, caregivers, and agency staff during overnight hours. This position requires strong clinical judgment, excellent communication skills, and the ability to respond appropriately to urgent patient needs while working independently. The ideal candidate is comfortable working nights, responding to after-hours calls, assessing patient needs, communicating with the appropriate clinical team members, and making timely decisions within their scope of practice. Essential Responsibilities Provide overnight on-call nursing support to patients and families. Respond promptly to after-hours patient and caregiver calls. Assess patient concerns and determine the appropriate nursing intervention. Provide clinical guidance and education to patients and caregivers within LVN scope of practice. Identify changes in patient condition and escalate concerns appropriately to the RN, physician, or other appropriate provider. Coordinate urgent visits when clinically necessary and within agency protocol. Communicate effectively with the on-call RN, physicians, hospice/home health leadership, and other members of the interdisciplinary team. Follow agency policies, procedures, and established after-hours protocols. Maintain accurate and timely documentation of all calls, interventions, communications, and patient-related activities. Assist with coordination of medications, supplies, equipment, and other patient needs as appropriate. Respond to patient emergencies according to agency protocol and scope of practice. Maintain patient confidentiality and comply with HIPAA requirements. Provide compassionate, respectful, and professional service to patients and families. Complete other nursing duties as assigned. Qualifications Current and active California Licensed Vocational Nurse (LVN) license. Current CPR/BLS certification. Valid California driver's license and reliable transportation. Current auto insurance and acceptable driving record. Ability to travel throughout Santa Rosa and surrounding communities when an in-person visit is required. Previous home health, hospice, skilled nursing, or community-based nursing experience preferred. Strong clinical assessment and communication skills. Ability to work independently with minimal supervision. Ability to remain calm and professional in urgent or stressful situations. Strong documentation and organizational skills. Must be dependable and responsive during scheduled overnight on-call hours. EMR / Documentation Experience with electronic medical records (EMR) systems preferred. Aura Home Health & Hospice utilizes Kinnser/WellSky as its electronic medical record (EMR) system. LVN must be comfortable completing timely and accurate documentation within the EMR, including patient communications, clinical notes, interventions, and other required documentation. Overnight On-Call Expectations The LVN must be available and reachable throughout the assigned overnight on-call shift.
The position may involve:
Receiving and returning patient/caregiver calls. Providing telephone triage and clinical support within scope. Escalating concerns to the appropriate RN, physician, or supervisor. Coordinating urgent patient needs. Making an in-person visit when required by the patient's condition and agency protocol. Documenting all overnight activity before the end of the assigned shift. This position requires flexibility and the ability to respond to urgent patient needs during overnight hours. Service Area The primary service area is Santa Rosa and surrounding cities in Sonoma County . Candidates should be comfortable traveling within the assigned service area when an in-person patient visit is required.
