    STANDBY RATE
    $150 for
    DAY/NIGHT ON-CALL PER VISIT
    $75/
    VISIT On-Call Hours:
    5:30pm

    Job Summary:
    Licensed Vocational Nurse (LVN) American Hospice and Home Health Services is currently seeking a Part Time, and/or Per Diem On-Call LVN to service San Joaquin County. Negotiable to salary or hourly pay rates depending on experience. Our home health care mission is to provide professional and paraprofessional services to patients in their homes, assisting them to achieve the highest level of potential in their day-to-day activities
    Essential Job Functions/Responsibilities:
    As the Hospice and Home Health LVN, you will provide nursing care and supportive services in accordance with the physician/provider treatment plan and the nursing process of assessment, diagnosis, planning, implementation and evaluation under the supervision of the Registered Nurse. Performs accurate assessments, interprets and correlates findings, establishes plan of care, observes patient progress, records pertinent observation, implements physician and nursing treatment plans and evaluates outcomes. Responsibilities and Duties Ability to complete all documentation in accordance with policy and procedure. Flexible, as varied daily activities will include phone, email and written communication, meetings with a variety of healthcare professionals, great understanding of hospice rules and regulations Must work closely with team members to provide the optimal care and services. Ability to manage scheduling visits, documentation time, and meetings effectively. Ability to learn an electronic medical records system.

    Requirements:
    Qualifications and Skills Licensed Vocational Nurse License in California Preferred 1-2 years Professional Experience in Home Health and Hospice Current CPR Certification IV Certification Wound Care and/or wound vac experience preferred Ability to drive within 30-60 miles a day Ability to pass drug and background screenings Ability to function autonomously Strong clinical skills Effective communication skills Proficient computer skills Reliable transportation and insurance
