Join our dedicated team at Millennium Home Health as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) . You'll play an essential role in supporting patient health and well-being under the supervision of a Registered Nurse. Key Responsibilities (include but are not limited to): Deliver direct, hands-on patient care in the home setting. Implement the plan of care initiated by the RN or provider. Ensure accurate and timely documentation in line with the plan of care. Educate patients and families/caregivers with information relevant to diagnoses and care plans. Coordinate with other members of the healthcare team and report needs to the RN or Clinical Supervisor. Use equipment and medical supplies effectively and efficiently. Participate in professional growth and development activities. Perform other duties as assigned by the supervising RN. Maintain patient confidentiality and adhere to HIPAA regulations. Supervise medical assistants when needed. Maintain a safe, clean working environment and follow infection-control protocols.

Qualifications:

Current and active LVN or LPN license in the state of California (Required) Minimum 1 year of nursing experience (Home health or medical/surgical experience preferred) Current CPR certification (Required) Strong verbal and written communication skills Highly organized with attention to detail Must be a licensed driver with a reliable, insured vehicle that meets state and company requirements Willingness and ability to travel approximately 30-50 miles from home for patient visits Complies with accepted nursing standards and ethical practices Why Join Millennium Home Health?
