POSITION SUMMARY
Affinity Hospice, Inc. is seeking a compassionate, dependable, and clinically skilled Licensed Vocational Nurse (LVN) to join our hospice team as a Field Nurse / Runner . The Hospice LVN Field Nurse/Runner provides direct patient care in the home, facility, and community settings while supporting the hospice clinical team with urgent field needs, medication and supply delivery, patient visits, symptom management, and other clinical responsibilities. The ideal candidate is comfortable working independently, responds well to changing priorities, demonstrates strong clinical judgment, and is committed to providing compassionate end-of-life care while maintaining the dignity, comfort, and safety of every hospice patient.
ESSENTIAL
RESPONSIBILITIESHospice Patient Care Provide skilled nursing care to hospice patients under the direction of the RN Case Manager and hospice plan of care. Perform routine, PRN, and urgent nursing visits as assigned. Assess patient condition, symptoms, comfort level, functional status, skin integrity, nutrition/hydration, and safety. Monitor for changes in condition and promptly communicate significant findings to the RN Case Manager, Clinical Coordinator, physician, and/or hospice interdisciplinary team as appropriate. Provide comfort-focused care consistent with the patient's hospice plan of care. Assist with management of common end-of-life symptoms including pain, dyspnea, anxiety, agitation, nausea/vomiting, secretions, constipation, and other changes associated with disease progression. Reinforce hospice education with patients, families, caregivers, and facility staff. Provide education regarding medications, comfort measures, fall prevention, infection prevention, skin care, nutrition/hydration, and when to contact hospice. Provide wound and skin care within the LVN scope of practice and established plan of care. Recognize signs of active dying and promptly communicate changes to the hospice RN and care team. Field Nurse / Runner Responsibilities Respond to assigned field needs in a timely and professional manner. Deliver hospice medications, comfort medications, supplies, DME-related items, paperwork, and other clinical materials as directed. Assist with obtaining and delivering medications from pharmacies when needed and permitted. Assist the clinical team with urgent patient needs, including PRN visits and symptom-related requests. Assist with obtaining patient supplies and ensuring needed items are available for patient care. Support admissions, transfers, discharges, and other field-based clinical activities as assigned. Assist with coordination of pharmacy, DME, facility, caregiver, and other hospice-related needs. Communicate effectively with facilities, caregivers, pharmacies, physicians, DME companies, and hospice staff. Assist with after-hours or weekend field needs when scheduled. Documentation & Communication Complete accurate, timely, and thorough clinical documentation in the hospice EMR. Document assessments, interventions, patient responses, education provided, and communication with the care team. Document PRN medication effectiveness and patient response appropriately. Report changes in patient condition promptly to the appropriate hospice clinician. Maintain confidentiality of patient information in accordance with HIPAA and hospice policies. Communicate effectively during handoffs and participate in interdisciplinary team coordination. Maintain accurate records of medications, supplies, deliveries, and other assigned field activities. Safety & Compliance Follow all hospice policies, procedures, infection-control standards, and applicable federal and state regulations. Practice within the LVN scope of practice and under appropriate RN supervision. Follow medication safety and controlled-substance procedures. Maintain patient and staff safety during all field visits. Report falls, injuries, medication errors, adverse events, abuse/neglect concerns, and other incidents according to hospice policy. Maintain current CPR certification and all required professional credentials. Complete required hospice, regulatory, and agency education.
QUALIFICATIONS
Required:
Current California LVN/LPN license in good standing. Current CPR certification. Hospice, home health, skilled nursing, long-term care, or related clinical experience preferred. Valid driver's license, reliable transportation, and current automobile insurance. Ability to travel throughout the hospice service area. Ability to work independently and prioritize multiple assignments. Strong communication and documentation skills. Ability to interact professionally and compassionately with patients and families experiencing end-of-life situations.
Preferred:
Previous hospice or palliative care experience. Experience with wound care and symptom management. Experience working with nursing facilities, assisted living facilities, or residential care settings. Familiarity with hospice medications and comfort-care protocols. Experience using electronic medical records.
PHYSICAL REQUIREMENTS
The LVN Field Nurse/Runner must be able to: Travel between patient locations throughout the assigned service area. Stand, walk, bend, stoop, and kneel as required for patient care. Assist with patient positioning and transfers within the limits of the employee's role and agency policy. Lift and carry clinical supplies and equipment as needed. Perform clinical assessments and hands-on patient care. Safely enter and work in private homes, facilities, and other community environments.

WORK ENVIRONMENT
This is a field-based position requiring travel to patient homes, skilled nursing facilities, assisted living facilities, residential care settings, hospitals, pharmacies, and other locations as needed. The LVN Field Nurse/Runner should be comfortable working independently while maintaining frequent communication with the hospice clinical team. This position may include routine, PRN, urgent, evening, weekend, or on-call assignments based on agency needs and the employee's scheduled availability.
