LVN POSITION SUMMARY

Perform professional nursing duties including assessments, treatment and follow through resolution of all health issues.

WORK LOCATION TYPE

Spends approximately 75% or more of time inside, providing care forindividuals withdevelopmental disabilities and complex behavioral/medical needs in a residential EBSH setting.

GENERAL DUTIES/RESPONSIBILITIES

Implements program policy and procedure according to program regulations and guidelines Provides overall medical support to individuals who receive services to include scheduling of medical appointments and follow up as needed. QUALIFICATIONS & EXPERIENCE LPN CA

license; valid driver's license; 1year experience working with individuals with developmental disabilities and/or mental health issues; experience working in a residential environment preferred Education/License/Certification(s) Associate Degree License Certification

Minimum Required 1•3 years
