In this vital role, you will deliver compassionate, high-quality nursing services directly in patients' homes, empowering individuals to achieve optimal health and independence. Your expertise will help bridge the gap between medical treatment and everyday living, ensuring personalized care that makes a real difference. This position offers an engaging opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those recovering from illness or injury. Responsibilities Administer medications following strict protocols and safety standards. Monitor vital signs, perform physical examinations, and evaluate patient responses to treatments. Assist with wound care, catheterization, tube feeding, and other specialized procedures using aseptic techniques. Educate patients and their families on health management, medication usage, and disease prevention strategies. Document all patient interactions accurately in Axxess, our electronic health record (EHR) system, while maintaining HIPAA compliance. Coordinate with physicians, case managers, and other healthcare providers to ensure seamless continuity of care. Skills Proven experience in acute care environments such as Medsrug or acute in-patient care. Familiarity with EMR/EHR system Axxess. Strong knowledge of physiology, anatomy, medical terminology, ICD coding (ICD-9/10), and diagnostic evaluation techniques. Skilled in patient assessment, vital signs monitoring, medication administration, and specimen collection/processing. Experience working with diverse populations including seniors with dementia or Alzheimer's disease; individuals with developmental disabilities; pediatric patients; and those requiring hospice or palliative care. Competency in IV insertion, catheter care, wound management, and infection control practices. Ability to perform physical examinations and procedures such as spinal taps or gastric lavage when needed. Excellent communication skills for triage assessments and health coaching; capable of working independently in home settings while adhering to safety standards.
