Job Summary We are seeking a dynamic and compassionate Wellness Coordinator/Licensed Practical Nurse (LPN) to join our dedicated healthcare team. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans. Administer medications, injections, IV infusions, and other treatments following strict safety protocols and medical documentation standards. Collaborate with interdisciplinary teams to develop and implement wellness programs tailored to individual needs, including memory care, dementia care, and chronic disease management. Provide education and coaching on health maintenance, medication management, nutrition, and lifestyle modifications to patients and their families. Maintain accurate electronic health records (EHR/EHR systems), ensuring compliance with HIPAA regulations and confidentiality policies. Familiarity with EMR/EHR systems such as August Health. Effectively communicate with staff, residents, families, medical providers and pharmacy. Transcribe new orders to EMR. Support Care coordinators and RA's as needed. Requirements Valid LPN license with current state licensure; additional certifications such as Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS) are preferred. Hands on patient care necessary. Candidate is expected to be on the floor as needed. Proven experience working with geriatric patients in long-term care, assisted living, or hospital settings; critical care or ICU experience is highly desirable. Strong understanding of medical terminology, anatomy knowledge, and physiology to support patient assessments effectively. Excellent communication skills for caregiver interaction, client education, and team collaboration; experience working with individuals with disabilities or developmental challenges is a plus. Ability to work independently while maintaining a high standard of clinical confidentiality and adhering to assisted living regulations.
