Job Description

As a Home Health LVN/LPN, youll practice at the top of your licensedelivering skilled, one-on-one care in patients homes. Youll have the autonomy to use your clinical judgment while making a real impact on patient outcomes and independence. Monitor assigned cases to ensure care is delivered per the Plan of Care, including tracking patient progress, identifying changes in condition, and reporting updates to the RN/physician.
