The LPN will work under the direction of a Registered Nurse and follow each patient's established plan of care. This is a PRN/per-diem position with a flexible schedule. Hours may range from 10 to 40 per week depending on patient needs and the nurse's availability.

This is a visit-based home-health position, not a continuous 8-12-hour shift. Nurses will travel to patient's home and complete individual visits based on patient's needs and availability. Responsibilities Provide skilled nursing care in patients' homes according to the established plan of care. Monitor vital signs, symptoms and changes in patients' conditions. Administer medications and treatments as ordered and within the LPN scope of practice. Provide wound care and other assigned skilled nursing services. Educate patients and caregivers about medications, treatments and safety precautions. Report changes in patients' conditions promptly to the supervising RN, physician and agency. Maintain accurate and timely visit notes and clinical documentation. Follow infection-control, patient-safety and home-health regulations. Communicate effectively with patients, caregivers and members of the care team. Participate in care coordination and staff meetings as required. Qualifications Current and unrestricted Colorado LPN license. Current CPR/BLS certification. Valid driver's license, reliable transportation and automobile insurance. Strong communication, assessment and documentation skills. Ability to work independently while following RN supervision and the established plan of care. Ability to travel to patients' homes. Home-health experience preferred, but not required. Must successfully complete all required background and credentialing checks.
