Yes We are seeking a compassionate and experienced LPN/LVN•Home Health for a 13-week contract assignment in Denver, Colorado. This opportunity is ideal for a dependable nurse who is comfortable working independently, providing quality patient care in a home health setting, and managing documentation accurately and efficiently. The ideal candidate will have strong clinical skills, prior home health experience, and proficiency with OASIS documentation and Homecare Homebase. Job Responsibilities Provide skilled nursing care to patients in their homes under the direction of an RN or physician Perform patient assessments and document care accurately in Homecare Homebase Assist with medication administration, wound care, treatments, and patient education Complete and maintain OASIS documentation in compliance with agency and regulatory standards Monitor patient progress and report changes in condition to the care team Coordinate care with interdisciplinary healthcare professionals Maintain compliance with infection control, safety, and home health protocols Float to different patient care areas or territories as needed Required Qualifications Minimum 2 years of experience as an LPN/LVN in a clinical setting Active Colorado LPN License or Compact LPN License required Current BLS Certification required Minimum 1 year of recent Home Health experience Minimum 1 year of OASIS documentation experience Minimum 1 year of experience using Homecare Homebase Strong communication, organization, and time management skills Ability to work independently in a home health environment Preferred Qualifications Previous travel or contract assignment experience Experience managing a daily patient visit schedule independently Knowledge of Medicare and home health compliance standards Why Join This Opportunity?
