Variable Night Shift (12-hour shifts)
Hours:
36 hours per week Shift Details 12-hour Night Shifts Variable scheduling 36 hours per week Position Summary "Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and long-term care setting. This role focuses on delivering safe, patient-centered care during overnight shifts, including medication administration, clinical monitoring, and coordination with the interdisciplinary care team. Key Responsibilities Administer medications and treatments as prescribed by physicians Monitor residents' conditions and promptly report any changes Perform routine nursing assessments and accurately document care provided Provide wound care and other basic nursing procedures within scope of practice Maintain complete and accurate clinical documentation and charting Assist with admissions, discharges, and transfers as needed Support Certified Nursing Assistants (CNAs) in resident care activities when required Ensure adherence to infection control and safety protocols at all times Respond to emergencies and resident needs during night shifts Collaborate effectively with the interdisciplinary healthcare team Required Qualifications Active and unrestricted LPN license (Colorado or compact state eligibility) Prior experience in long-term care or skilled nursing preferred Ability to work consistent overnight 12-hour shifts Strong clinical judgment and documentation skills Ability to work independently and manage patient care during night shifts For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
