This is a true clinical leadership role for an RN or LPN who knows Colorado Assisted Living Residence (ALR) and Memory Care regulations and has a heart for dementia and Alzheimer's care. What You'll Do as Health Services Director The Health Services Director oversees the clinical and resident care program for our Memory Care community. You'll assess residents, build individualized service plans, coordinate care with families and outside providers, and mentor a team of medication aides and care partners. Every day in this dementia care leadership role, you will: Create a resident-centered approach to planning and delivering services
  • supporting residents' dignity, choice, and right to participate in their own care.
Perform initial and ongoing assessments of health, functional, cognitive, and psychosocial status; develop and update individualized service plans (ISPs) for each Memory Care resident. Proactively manage residents with changes in condition, partnering with the management team to keep residents in the least restrictive, most appropriate level of care. Collaborate on move-in decisions
  • conducting pre-move-in assessments and balancing prospective resident needs against community acuity and staffing.
Coordinate with physicians, hospice, home health, discharge planners, case managers, and state agency personnel; arrange pharmacy, podiatry, lab, and x-ray services. Communicate regularly with families on care updates, end-of-life conversations, and dementia education. Supervise, train, and develop care partners, medication aides (QMAPs), and Resident Care Coordinators; delegate nursing tasks in accordance with the Colorado Nurse Practice Act. Ensure regulatory compliance with Colorado Department of Public Health and Environment (CDPHE) Assisted Living Residence and Secure Environment / Memory Care rules (6 CCR 1011-1, Chapter 7); lead survey readiness and corrective action plans. Manage clinical systems, infection prevention, medication management, fall prevention, and behavioral health programs specific to dementia and Alzheimer's care. What You Bring Active, unencumbered Colorado nursing license: RN (Registered Nurse) or LPN (Licensed Practical Nurse). Either license is welcome
  • the right leader matters more than the letters after the name. Working knowledge of Colorado Assisted Living and Memory Care regulations (6 CCR 1011-1 Chapter 7, Secure Environment requirements, QMAP delegation, ISP standards). This is essential. Dementia and Alzheimer's expertise
  • hands-on experience in a Memory Care, secured neighborhood, or skilled dementia unit.
Leadership experience:
supervising care staff, coaching unlicensed personnel, managing budgets, and driving quality outcomes. Strong assessment, case management, and care planning skills. Compassionate communicator with residents, families, physicians, and team members.
Preferred:
prior experience as a Director of Nursing (DON), Resident Care Director, Wellness Director, Assistant Director of Nursing (ADON), Nurse Manager, MDS Coordinator, or Charge Nurse in assisted living, memory care, or long-term care. About Aspen Ridge Memory Care
