Job Description

Active Colorado LPN License Required Certification:
BLS Required Job Responsibilities:
Provide direct nursing care to LTC/SNF patients. Assess, plan, implement, and coordinate patient care. Monitor patient conditions and report changes appropriately. Administer medications and treatments as ordered. Maintain accurate patient documentation. Collaborate with nurses, physicians, patients, and families. May provide coverage at Granby MedSurg as needed.
Requirements:
Active Colorado LPN License Current BLS Certification LTC/SNF experience preferred Must be available for 2 call shifts every 6 weeks Overtime may be required based on facility needs. #Nurse2026
Pay:
$1,500.00 - $1,630.00 per week
