- LPN•TRU•Hospice•Central Staffing TRU Community Care•3.
- $33.
Position:
PRN- Licensed Practical Nurse (LPN)
Schedule:
PRN- Shift coverage includes nights and Wknd, varied hours but 12 hour shifts
Location:
Lafayette, CO Pay:
$26.67- 33.
EDUCATION AND QUALIFICATIONS
- Must possess a current LPN license in the state of Colorado.
- Must have minimum one year LPN experience
- hospice or home health experience preferred.
- Must successfully pass agency required background checks.
- Must possess a valid driver's license, provide proof of insurance, and have reliable transportation.
- Must comply with agency health screening requirements.
- Must possess CPR certification or be able to obtain within 60 days of hire.
- Must be able to demonstrate computer competency.
ESSENTIAL DUTIES
- Collects data regarding all body systems, signs and symptoms, deviations, acuity, vital signs, and responses in collaboration with the Registered Nurse, primary care physician, and/or hospice physician contributing to the individualized plan of care.
- Incorporates patient/family preferences and goals within the plan of care.
- Supports and respects the patient's and family's unique diversity, including spiritual and cultural beliefs.
- Completes documentation in an accurate and complete manner as per agency standard and in compliance with regulatory standards.
- Provides patient and family education as appropriate. Areas of education include, but are not limited to personal care, medications, procedures, safety, diet and nutrition, disease process, death and dying.
- Contributes to the team, utilizes team resources, supports team decisions, and maintains positive working relationships with all team members.
- Demonstrates effective verbal, written, and electronic communication skills. Gives and receives feedback in a timely open manner. Engages collaboratively in conflict resolution processes as needed.
- Is flexible and responsive in meeting and exceeding the expectations of all customers, both internal and external.
- Participates in mandatory educational activities and required meetings.
