•We are seeking a dedicated and energetic Licensed Practical Nurse (LPN) to join our team during the night shift. Duties Deliver comprehensive nursing care to residents during overnight hours, including medication administration, vital signs assessment, and patient assessments. Monitor residents' conditions closely, recognizing signs of acute pain, infection, or deterioration, and respond promptly with appropriate interventions. Assist with activities of daily living such as tube feeding, catheter care, and mobility assistance while maintaining patient dignity and comfort. Document all patient interactions accurately using Electronic Health Records (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and confidentiality policies. Collaborate with interdisciplinary teams including physicians, case managers, and therapists to develop and implement individualized care plans. Manage emergency situations efficiently by utilizing airway management skills, first aid knowledge, and critical care experience in settings such as ICU or trauma centers. Support residents with dementia or Alzheimer's disease through specialized memory care techniques and behavioral health strategies. Requirements Valid state licensure as a Licensed Practical Nurse (LPN). Proven experience working in long-term care facilities, skilled nursing facilities, or assisted living environments. Strong knowledge of medical terminology, anatomy, physiology, and clinical procedures such as IV infusion and venipuncture. Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Experience with critical care areas such as ICU or emergency medicine is highly desirable. Ability to perform physical examinations and vital signs assessment accurately while adhering to infection control protocols. Excellent communication skills for effective caregiver communication and client interaction across diverse populations including geriatric patients and individuals with disabilities.
