Job Summary Join our dedicated healthcare team as a Night Shift Licensed Practical Nurse (LPN) or Registered Nurse (RN), where you will play a vital role in providing exceptional patient care during overnight hours. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to inform care plans. Administer medications, including injections and IV infusions, following strict protocols and documentation standards. Manage complex care needs such as Alzheimer's, dementia, memory care, and patients with developmental disabilities. Utilize EMR (Electronic Medical Records) systems like Epic or Cerner to accurately document patient information, treatment plans, and progress notes. Monitor patients' conditions closely, managing acute pain, airway management, ventilator support, and infection control measures as needed. Collaborate with interdisciplinary teams to coordinate discharge planning, case management, and follow-up care for long-term or post-acute patients. Ensure compliance with HIPAA regulations and maintain clinical confidentiality policies at all times. Skills Proven experience working with diverse patient populations in settings such as long-term care facilities, hospitals (including ICU and trauma centers), assisted living communities, or home care environments. Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10) for accurate documentation and billing purposes. Skilled in performing medical procedures including venipuncture, catheterization, suturing, specimen collection/processing, and sterile techniques. Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for efficient record management. Ability to handle emergency situations confidently with first aid skills and basic life support (BLS) certification; experience in acute care or emergency medicine is highly valued. Excellent caregiver communication skills to effectively educate patients and collaborate with families while respecting clinical confidentiality policies. Experience working with individuals with disabilities or special needs such as Alzheimer's or dementia care is preferred; familiarity with hospice or palliative care is a plus.
