Willow Tree Care Center is seeking a dedicated and compassionate full-time Licensed Practical Nurse (LPN) to join our dynamic team. $26.00- $36.00 per hour. Pay is dependent on applicant's relevant experience. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans. Administer medications and injections accurately, including IV infusions and catheter care, following strict protocols and safety guidelines. Manage complex medical documentation using Electronic Health Record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA and confidentiality policies. Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures to support accurate diagnoses. Provide specialized care for patients with dementia, Alzheimer's disease, memory impairments, or developmental disabilities, tailoring interventions accordingly. Support acute pain management and emergency response efforts within hospital or urgent care settings, including airway management and first aid. Collaborate with multidisciplinary teams in settings such as skilled nursing facilities, assisted living communities, hospitals, or home care environments to coordinate comprehensive patient care. Qualifications Valid Licensed Practical Nurse (LPN) license in good standing with the state licensing board. Proven experience working with diverse patient populations including geriatrics, pediatrics, or individuals with disabilities. Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10). Experience with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for accurate documentation. Competency in medication administration techniques including injections, venipuncture, and IV insertion. Ability to perform physical examinations and vital sign assessments efficiently and accurately. Familiarity with infection control practices and aseptic techniques essential for safe patient interactions.
