LPN - SNF 3

x 12 Day shifts. 36/48 schedule36/48 contract. We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide essential nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. This role involves delivering high-quality patient care, supporting medical teams, and ensuring compliance with healthcare regulations. The ideal candidate will possess a strong foundation in patient assessment, medication administration, and clinical procedures, along with experience working with diverse patient populations, including geriatric and individuals with disabilities. Duties Perform comprehensive patient assessments, including vital signs and physical examinations, to inform care plans. Administer medications and injections accurately following prescribed protocols while monitoring for adverse reactions. Assist with invasive procedures such as catheterization, venipuncture, and sterile dressing changes. Manage patient documentation using electronic health record (EHR) systems like Epic or Cerner to ensure accurate medical records. Provide wound care, including dressing changes and infection control measures, adhering to aseptic techniques. Support patients with activities of daily living (ADLs), including mobility assistance and tube feeding when necessary. Collaborate with multidisciplinary teams to develop and implement individualized care plans. Educate patients and their families on health management strategies, including medication adherence and disease-specific care such as Alzheimer's or dementia. Participate in triage activities by assessing urgent needs and prioritizing care in emergency or urgent care settings. Ensure compliance with HIPAA regulations and maintain strict confidentiality of patient information. Skills Proven experience working in diverse healthcare environments such as hospitals (including ICU, trauma centers), nursing homes, hospice, or outpatient clinics. Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Strong knowledge of medical terminology, physiology, anatomy, and pharmacology related to nursing practice. Skilled in medication administration, IV infusion techniques, catheterization, phlebotomy, and basic life support (BLS). Experience managing complex cases involving acute pain management, infectious diseases, geriatrics, or behavioral health conditions. Ability to perform clinical procedures such as suturing, gastrostomy feeding tube management, and airway management. Familiarity with long-term care regulations and documentation standards for skilled nursing facilities. Excellent communication skills for caregiver coordination and patient education across diverse populations. Knowledge of ICD coding (ICD-9/10) for accurate medical record keeping and billing processes. Ability to work effectively under pressure in fast-paced environments such as emergency rooms or trauma centers.
