Travel LPN for a skilled nursing/long-term care facility in Akron, Colorado. The role covers day or night shifts on a 3x12-hour schedule with rotating weekends. The assignment supports patient care in a skilled nursing environment.

Key Responsibilities:

Assess patient health status and update care plans Administer medications and treatments per protocol Document all patient interactions and changes Collaborate with interdisciplinary team to coordinate care Monitor vital signs and report abnormalities Assist patients with daily activities and mobility Provide wound care and hygiene support Maintain infection control standards Communicate effectively with patients and families Adapt to changing patient needs and shift demands Ensure accurate and timely charting Support staff during shift transitions

Qualifications:
