Job Summary We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality, patient-centered nursing care across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. This role requires a proactive approach to patient assessment, medication administration, and documentation, ensuring compliance with healthcare regulations and standards. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to enhancing patient well-being through attentive care. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Monitor patients for signs of acute pain or distress and implement appropriate interventions. Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures. Maintain detailed medical records using Electronic Health Record (EHR) systems while ensuring compliance with HIPAA and confidentiality policies. Support discharge planning and coordinate care with multidisciplinary teams for optimal patient outcomes. Provide specialized care for individuals with dementia, Alzheimer's disease, or other cognitive impairments in memory or hospice care settings. Manage airway devices such as ventilators when necessary and perform emergency procedures like first aid or CPR. Educate patients and families on health management topics including medication administration, nutrition, and chronic disease management. Ensure infection control practices are followed rigorously to prevent the spread of infectious diseases. Collaborate effectively with physicians, case managers, caregivers, and other healthcare professionals to deliver comprehensive care. Skills Proven experience working as an LPN in diverse healthcare environments such as hospitals, nursing homes, or home care settings. Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth for accurate documentation. Strong knowledge of medical terminology, physiology, anatomy, and nursing practices. Ability to perform venipuncture (phlebotomy), catheterization, tube feeding management, and sterile techniques. Experience with critical care areas including ICU or Level I Trauma Centers is highly desirable. Skilled in triage assessment to prioritize patient needs effectively in urgent or emergency situations. Familiarity with ICD coding (ICD-9/ICD-10), CPT coding for procedures, and documentation standards. Excellent communication skills tailored to working with diverse populations including geriatric patients and individuals with disabilities. Knowledge of long-term care regulations and best practices in geriatrics and dementia care. Ability to work independently while maintaining a team-oriented approach in fast-paced environments.
