2996671071-2 Apply for this job now Job Description Hartford Healthcare seeks a compassionate LPN (Licensed Practical Nurse) to deliver high-quality, patient-centered care within our Healthcare & Medical Services team. In this role, you will provide direct clinical support, administer medications and treatments, monitor patient status, and accurately document care in our electronic medical record. You will collaborate closely with RNs, physicians, and the care team to ensure safe, evidence-based practice while educating patients and families. Responsibilities Provide direct patient care under RN/physician direction, including vital signs, medication administration, and treatments Assist with patient assessments, care planning, and documentation in electronic medical records Support physicians and RNs during exams, procedures, and interventions Educate patients and families on treatment plans, medications, and self-care Maintain infection control, safety, and regulatory compliance standards Coordinate with interdisciplinary team to ensure high-quality, compassionate care Monitor patient status and promptly report changes to supervising clinicians Prepare and maintain exam/treatment rooms, equipment, and supplies Required Skills Medication administration and dosage calculation Wound care and basic dressing changes Vital signs monitoring and patient observation Electronic medical record (EMR) documentation Infection control and safety protocols Phlebotomy and specimen collection IV administration (per state scope and certification) Patient and family education Basic EKG monitoring (preferred) Clinical triage and prioritization
