The Licensed Practical Nurse works under the direction of a registered nurse to provide nursing care, to assist the patient in learning self-care techniques and to prepare clinical and progress notes. Responsibilities 1. Assesses patients within scope of practice at every home visit.
2. Carries out treatments per MD orders.
Reports off to RN case manager and/or clinical manager.
3. Reports all changes in condition to MD.
4. Provides patient/caregiver education as per care plan.
5. Completes all documentation within 24 hours. Must include changes in condition, follow-up with provider or
other disciplines with supporting documentation.
6. Attends all staff meetings, inservices, and all other meetings as required.
7. Compliance with all mandatory requirements -inclusive of Healthstreams.
8. Follows policies and procedures. Qualifications
EDUCATION
Graduated from an approved Practical Nurses Education Program
EXPERIENCE
Minimum one year experience in the acute care setting. Home care experience preferred, but not required.
LICENSURE
License Practical Nurse with valid (or eligible) Connecticut license required.
A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day.
CPR certification is required and must be maintained. Candidates who do not have certification are required to obtain it within 3 months (90 days) of hire.
SPECIAL SKILLS
Excellent verbal and written skills.
Excellent clinical knowledge.
Strong computer skills.
PHYSICAL DEMAND
Ability to perform tasks involving physical activity, which includes heavy lifting, extensive bending, administering CPR, patient bracing, standing, and stairs.
Requires exposure to blood and body fluids, hazardous materials, and communicable diseases. YNHHS Requisition ID 191937
