Job Overview We are seeking a dedicated, compassionate, and experienced and Flexible Licensed Practical Nurse (LPN) to join our care team. In this vital role, you will provide high-quality nursing care under the supervision of a Registered Nurse (RN) in a home setting. ResponsibilitiesProvide direct patient care in accordance with the established plan of care and physician orders.
Monitor and document vital signs, patient status, and changes in condition.
Administer medications safely and accurately, including: Gastrostomy Tube (G-Tube)Nebulizer treatmentsRectal medicationsOther prescribed routes of administrationPerform routine nursing assessments and observations.
Monitor, observe, and report concerns related to G-Tube function, complications, or signs of infection.
Maintain timely, accurate, and thorough clinical documentation.
Assist with activities of daily living (ADLs), including: Bathing and personal hygieneDressing and groomingPositioning and mobility assistanceFollow all agency, home care, state, and insurance regulations, policies, and procedures. Communicate effectively with the interdisciplinary care team, physician, patient, and family.
Respond appropriately to changes in patient condition and emergency situations. QualificationsRequiredCurrent, unrestricted state licensure as a Licensed Practical Nurse (LPN). Minimum of 3 years of pediatric nursing experience.
Experience caring for patients with G-Tubes.
Strong clinical documentation skills are important.
Excellent communication, observational, and critical-thinking abilities. Ability to work independently while maintaining collaboration with the supervising RN and healthcare team.
Active CPR certification Schedule Requirements Current openings are primarily for overnight (night shift) coverage. Additional opportunities for morning and evening shifts may become available based on patient and staffing needs. Candidates should be willing to work overnight hours if needed and maintain a degree of flexibility to accommodate schedule changes and additional shift opportunities when needed. Reliability, punctuality, and the ability to adapt to varying schedules are essential for success in this role. PreferredExperience in one or more of the following settings:
