This position involves providing comprehensive nursing care, conducting physical examinations, and ensuring compliance with health regulations. The Wellness Nurse will work collaboratively with our team to develop individualized care plans and support the overall well-being of the residents wihtin our community. The requirements listed throughout this job description are representative of the knowledge, skills, and abilities required. Graduate of accredited college/school of nursing Maintains current state license as a professional Registered Nurse (RN) or Licensed Practical Nurse/Licensed Minimum year experience in home health, assisted living, hospital, or long term care environment Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care; federal, state, and local regulations Demonstrated knowledge of good assessment skills Ability to handle multiple priorities Possess written and verbal skills for effective communication and level of understanding Competent in organizational, time management skills Demonstrates good judgment and problem solving skills Ability to work weekends, evenings, and flexible hours; available to our residents at peak service delivery days and times
Essential Duties:
Identifies on-going needs and services of residents in order to provide the highest quality of service Ensures proper follow through, assessment, and documentation for residents with a change in healthcare services and status Assists Director of Nursing (DON) in conducting resident assessments at time of move in and continually going forward Conducts monthly wellness visits for residents Documents all pertinent information in the resident wellness file including pertinent information located in the daily log Maintains communication with resident's family members to ensure medical needs for the resident are being met and communicates outcomes of monthly wellness visits as needed Contacts resident's attending physician when necessary and/or upon family request Assists in maintaining wellness files according to policies, federal, and state regulations Maintains medical supplies for community and emergency kits; orders medical supplies as directed by DON Provides clinical support and assistance to community team members as needed Demonstrated practices and guidelines for care and services surrounding resident's skin, nutrition, behavior, falls, and bowel/bladder Monitors the medication room, med charts, treatment carts for neatness, cleanliness, availability of medications, and expired medications Ensures established safety procedures are being followed at all times Maintains universal care precautions of cleanliness, hygiene, and health standards This role is essential for fostering a supportive environment that enhances the quality of life for patients while promoting their overall health and wellness.
