Job Summary We are seeking a skilled and dedicated LPN/RN Home Health Nurse to provide comprehensive in-home patient care. This full-time position is ideal for professionals experienced in managing complex medical needs, including feeding tubes, specialized durable medical equipment (DME), medication administration, and ensuring patient safety. The role involves delivering high-quality, personalized care to patients with diverse health conditions, emphasizing safety and comfort in their home environment. Schedule - Wednesday, Friday, Saturday, Sunday Nights 10p-10am Duties Perform detailed patient assessments and develop individualized care plans tailored to each patient's needs. Administer medications accurately and monitor patient responses to ensure safety and effectiveness. Manage and operate specialized DME, including feeding tubes and other medical devices, ensuring proper use and maintenance. Provide skilled nursing interventions related to ALS and other complex health conditions, offering expert care for high-profile cases. Educate patients and their families on medication management, use of medical equipment, safety protocols, and overall health maintenance. Monitor patient progress regularly, document clinical observations meticulously, and communicate updates to the healthcare team. Ensure compliance with all safety standards and healthcare regulations during home visits. Collaborate with physicians, therapists, and other healthcare professionals to coordinate comprehensive patient care. Skills Valid LPN or RN license with current state certification. Proven experience in home health nursing, especially with complex cases such as ALS or patients requiring feeding tubes and specialized DME. Strong knowledge of medication administration, safety procedures, and infection control practices. Ability to operate and troubleshoot specialized medical equipment safely. Excellent assessment skills with attention to detail in documenting patient conditions. Compassionate communication skills for educating patients and families effectively. Ability to work independently while maintaining a high standard of professional accountability.
