Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history reviews to determine individual care needs. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Utilize Electronic Health Record (EHR) systems such as Epic or Cerner to document patient information, treatment plans, and medical records with precision and confidentiality. Provide specialized care for patients with complex conditions such as Alzheimer's disease, dementia, or those requiring memory care or hospice services. Manage acute pain through appropriate interventions and monitor patient responses to ensure comfort and safety. Collaborate with multidisciplinary teams to develop discharge planning strategies and coordinate post-acute or home care services. Conduct diagnostic evaluations, specimen collection, and processing while adhering to infection control standards and sterile techniques. Support patients across various environments including medical-surgical units, ICU, emergency rooms, pediatric units, and skilled nursing facilities. Demonstrate proficiency in advanced procedures such as tube feeding, ventilator management, suturing, phlebotomy, and airway management. Ensure compliance with long-term care regulations, HIPAA privacy policies, and clinical confidentiality policies while maintaining accurate medical documentation. Skills Proven experience working with diverse patient populations such as geriatric patients, individuals with developmental disabilities, or those in acute or chronic care settings. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), CPT coding, and documentation standards within EMR/EHR systems like Epic or Cerner. Skilled in medication administration including injections and IV infusions; capable of performing venipuncture and catheterization procedures. Experience in critical care environments such as ICU or Level I/II trauma centers; familiarity with emergency medicine protocols is highly desirable. Ability to perform physical examinations, diagnostic evaluations, and patient assessments efficiently in fast-paced settings like urgent care or hospital medicine. Excellent caregiver communication skills with the ability to educate patients and families on health coaching, discharge instructions, and medication management. Knowledge of infection control practices, sterile processing techniques, aseptic methods, and infection prevention policies. Competency in using EMR/EHR systems for medical records management while maintaining compliance with HIPAA regulations. Familiarity with specialized areas such as neonatal ventilator management or pediatric nursing is a plus; experience working with individuals with disabilities is highly valued.
