Job Summary We are seeking a dedicated and skilled RN/LPN Registered Nurse or Licensed Practical Nurse to join our team. The ideal candidate will possess a strong clinical background, a passion for patient-centered care, and experience working with diverse populations. In this role, you will provide comprehensive nursing services in patients' homes, ensuring safe, effective, and compassionate care while adhering to all regulatory standards and organizational policies. Duties Deliver skilled nursing care to patients across various settings, including long-term care, assisted living, hospice, and acute care environments. Perform thorough patient assessments, vital signs monitoring, and physical examinations to evaluate health status and develop individualized care plans. Administer medications, injections, IV infusions, and perform wound suturing with precision and adherence to aseptic techniques. Manage complex medical procedures such as catheterization, dialysis support, ventilator management, and tube feeding. Document all patient interactions accurately using EMR systems like Epic or Cerner while maintaining HIPAA compliance. Collaborate with physicians, case managers, families, and interdisciplinary teams to coordinate comprehensive care plans. Provide education on disease management, medication administration, and health coaching tailored to patient needs. Support patients with developmental disabilities or memory care requirements by implementing specialized approaches aligned with NCQA standards. FLN Skills Proficiency in electronic health record (EHR) systems such as Epic, Cerner, or eClinicalWorks. Extensive experience with critical care procedures including ICU or trauma center experience preferred. Knowledge of infectious disease care, trauma medicine, and acute pain management techniques. Strong understanding of medical terminology, ICD coding (ICD-9/10), CPT coding, and laboratory specimen collection/processing. Ability to perform advanced skills such as sonography, gastrostomy management, ostomy care, and sterile processing. Excellent patient assessment skills with a focus on physiology knowledge and diagnostic evaluation capabilities. Experience working with diverse populations including pediatrics (neonatal ventilator), geriatrics (memory/dementia care), and individuals with disabilities or developmental challenges. Demonstrated ability to work in fast-paced environments such as emergency rooms or Level I Trauma Centers while maintaining high standards of safety and quality.
