•PAY
RATE IS CASE BY CASE BASIS
• As a Skilled Nurse Home Care Agent , you will provide in-home nursing care to patients throughout Broward County. You will visit patients independently, with full trust and autonomy, while maintaining a strong connection with the broader care team. The role requires a proactive, compassionate professional who can provide high-quality, personalized care for patients with complex needs in a variety of settings.
Key Responsibilities:
Provide in-home care: Conduct home visits to assess and manage patient health needs, including administering medications, wound care, and other skilled nursing tasks as required.
Patient Monitoring:
Monitor patient progress, identify health concerns, and adjust care plans accordingly. Ensure proper documentation of patient condition and treatment.
Patient and Family Education:
Educate patients and their families on care plans, medication management, and lifestyle adjustments to improve overall health and well-being.
Care Coordination:
Collaborate with physicians, caregivers, and the rest of the care team to ensure comprehensive, high-quality care for each patient.
Health Assessments:
Conduct thorough assessments of patient's health conditions and respond to changes in health status, ensuring timely interventions.
Emergency Response:
Provide immediate care in case of emergencies or complications, ensuring patients' safety and well-being at all times.
Record Keeping:
Maintain accurate patient records, including progress notes, treatment plans, and any changes in medical condition, ensuring compliance with healthcare regulations.
Qualifications:
Licensing & Certifications:
Active Registered Nurse (RN) license in the state of Florida. Current CPR certification . Valid Driver's License and reliable transportation. Ongoing CEU certifications as required by the state.
Experience:
(Prefered) Minimum of 1-2 years of nursing/agency experience, preferably in-home care or a similar setting (e.g., hospital, rehabilitation, or long-term care). Experience in managing patients with complex medical needs is a plus.
Skills & Attributes:
Strong clinical skills and ability to work independently in a home care setting. Excellent interpersonal skills, with the ability to communicate effectively and empathetically with patients, families, and team members. Ability to assess patient needs and adjust care plans proactively. Highly organized with attention to detail and the ability to document accurately. A compassionate approach to patient care, with a commitment to improving the quality of life for individuals in your care. Self-motivated, reliable, and able to handle a flexible work schedule with some evening or weekend shifts as needed. Why Join Us?
