Job Summary Join our dynamic healthcare team as a Licensed Practical Nurse (LPN) in our Call Center Triage department! In this vital role and under the direction of the RN you will provide guidance to patients over the phone, ensuring they receive timely and appropriate care. Duties Answer dedicated office sick line and document patient's concerns including vital signs, symptom evaluation, and medical history review Provide clear, accurate triage recommendations based on clinical protocols, including advice on medication administration, wound care, IV infusion guidance, and emergency response procedures. Collaborate with healthcare providers and emergency services when urgent or complex cases arise, ensuring seamless communication and appropriate escalation. Document all patient interactions meticulously in medical records while maintaining strict adherence to HIPAA and clinical confidentiality policies. Assist with intake processes for new patients and follow-up calls for ongoing care management in areas such as geriatrics, dementia care, hospice support, or pediatric needs. Educate patients and caregivers on health management strategies including nutrition, medication administration, physical examinations, and infection control practices. Skills Valid LPN licensure with active state license; experience working within EMR/EHR systems. Critical care experience across settings such as ICU, emergency room, or Level I Trauma Center enhances triage capabilities. Demonstrated proficiency in medical documentation and diagnostic evaluation techniques. Strong knowledge of physiology, anatomy, pharmacology related to pain management, infectious disease care, and acute care procedures such as catheterization or sterile processing. Excellent caregiver communication skills with experience working with diverse populations including individuals with chronic care conditions Ability to perform patient assessments involving vital signs collection, physical examinations, specimen collection/processing, and basic life support (BLS). Knowledge of HIPAA regulations and clinical confidentiality policies to ensure patient privacy at all times.
