Home Health Licensed Practical Nurse (LPN) - Night Shift Position Summary We are looking for a skilled and patient-focused LPN to provide in-home clinical care. This role involves delivering individualized nursing services, promoting patient safety, and supporting positive health outcomes in a home-based setting. The ideal candidate is organized, confident in independent practice, and committed to providing compassionate care. Key Responsibilities Complete thorough nursing assessments and create personalized care strategies tailored to each patient. Deliver physician-ordered treatments, medications, and skilled nursing interventions. Assess and track vital signs, symptoms, and overall health status, reporting significant changes promptly. Educate patients and caregivers on diagnosis, medication management, and preventive care. Maintain timely and accurate clinical documentation within the agency's electronic charting system. Support patients with mobility needs and other health-related assistance as appropriate. Qualifications & Skills Active LPN license in the state of Florida. Strong clinical assessment and critical-thinking skills. Experience in home health or community-based care preferred. Knowledge of infection control standards and emergency response procedures. Effective communication skills and ability to build rapport with patients and families. Ability to manage time efficiently and work independently in the field. Knowledge with tube feedings and seizure protocols.
