Provide skilled nursing care based on the plan of care and physician orders Document clinical and progress notes accurately Teach patients and families self-care techniques Perform preventive and rehabilitative nursing procedures Monitor and report changes in patient condition Participate in care conferences with the healthcare team Maintain confidentiality and follow agency policies

Qualifications:
