Overview Client Care Coordinator - Experienced Applicants Only Comfort Keepers is seeking an experienced, compassionate, and highly organized Client Care Coordinator to oversee client services and help ensure exceptional care delivery. We also welcome qualified LPNs with relevant home care or care coordination experience to apply. The Client Care Coordinator serves as the primary liaison among clients, families, caregivers, schedulers, and the Human Resources Coordinator. Responsibilities include meeting prospective clients, establishing client records, assessing service needs, completing monthly client visits, providing service quotes, scheduling shifts, matching caregivers with clients, supervising caregivers, and following up to ensure satisfaction with services. The ideal candidate is a dependable self-starter and effective problem-solver with outstanding communication, organization, and customer service skills. Applicants must have at least two years of related experience or an equivalent combination of education and experience. Previous home care, healthcare, or client care coordination experience is required. Candidates must be proficient in Microsoft Office, Excel, PowerPoint, multiline telephone systems, and general office equipment. This position requires local travel in a personal vehicle, participation in an on-call rotation, and occasional availability during evenings, late nights, and holidays as needed. A high school diploma or GED is required. LPN licensure is welcomed but not required. Responsibilities Coordinate patient care plans by collaborating with multidisciplinary healthcare professionals to ensure comprehensive treatment delivery. Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to inform clinical decision-making. Manage electronic health records (EHR) and medical records with accuracy, ensuring compliance with HIPAA regulations and privacy standards. Facilitate discharge planning and post-acute care arrangements, including hospice or home health services, to support smooth transitions for patients. Perform intake assessments for new patients, evaluating their needs and developing personalized care strategies. Monitor patient progress through ongoing clinical case management, adjusting plans as necessary based on diagnostic evaluations and patient responses. Engage in utilization management by reviewing medical necessity and supporting appropriate resource allocation within managed care frameworks. Maintain up-to-date knowledge of health regulation policies, medical terminology, and hospital protocols to ensure high-quality patient service. Communicate effectively with clients and their families to provide education, address concerns, and promote understanding of treatment plans. Collaborate closely with healthcare professionals across inpatient, outpatient, emergency room, ICU, PICU (Pediatric Intensive Care Unit), and other specialized units to optimize patient outcomes. Qualifications Proven expertise in patient assessment, infectious disease care, geriatrics, behavioral health, or pediatrics. Strong familiarity with electronic health records (EHR/EHR) systems and medical terminology. Knowledge of health regulation policies including HIPAA compliance and healthcare privacy standards. Experience managing complex cases within acute or post-acute care environments such as hospice or home health services. Ability to collaborate effectively with multidisciplinary teams while demonstrating excellent communication skills. Demonstrated understanding of physiology principles relevant to diverse patient populations across inpatient and outpatient settings. Prior experience in discharge planning, diagnostic evaluation support, utilization management, or managed care is highly desirable. Able to pass a drug test.
