This is a part-time hourly field position designed for a nurse who is comfortable managing multiple patients during the workday and providing efficient, patient-centered care in the home setting. The LPN will provide authorized skilled nursing services under the direction of the RN and patient's plan of care. The nurse is expected to complete clinical documentation at the point of care whenever practicable, allowing assessment, treatment, education, care coordination, and documentation to occur as part of an organized home visit. Primary Responsibilities Provide skilled nursing services in patients' homes in accordance with the established plan of care and physician/provider orders. Perform routine nursing assessments and promptly report changes in patient condition to the supervising RN and appropriate members of the care team. Provide wound care, dressing changes, medication-related services, treatments, and other skilled nursing interventions within the LPN scope of practice. Monitor patient response to treatment and identify concerns requiring RN or physician escalation. Provide patient and caregiver education regarding medications, treatments, disease management, safety, and the plan of care. Complete accurate, thorough, and timely clinical documentation, with an emphasis on point-of-care documentation during the home visit. Accurately complete required EVV, visit verification, timekeeping, and other agency documentation. Follow assigned patient schedules and communicate delays, cancellations, changes in condition, or scheduling concerns promptly. Coordinate with RNs, therapists, care coordinators, physicians, and other members of the interdisciplinary team. Follow infection-control, patient-safety, HIPAA, and WHCC clinical policies and procedures. Participate in supervisory reviews, competency assessments, clinical education, and quality-improvement activities as required. Scheduling & Field Expectations This is an hourly field-based position. The LPN may see multiple patients during an assigned workday. Patient assignments will be organized geographically whenever possible to reduce unnecessary travel and create efficient clinical schedules.

The nurse must:

Begin and end patient visits accurately using the agency's required systems. Complete documentation during the patient encounter whenever reasonably possible. Maintain accurate records of work and required travel between assigned patients. Follow WHCC procedures regarding travel time, personal breaks, mileage, and timekeeping. Be comfortable traveling between patient homes throughout the assigned territory. Maintain reliable transportation, a valid driver's license, and required automobile insurance. Service Area Seeking LPN coverage in Seminole, Orange, Osceola, Brevard, and Volusia Counties. Clinicians are not expected to cover all five counties; assignments will be based on preferred territory, availability, and patient proximity. Minimum Qualifications Current, active Florida Licensed Practical Nurse (LPN) license in good standing. Current BLS/CPR certification. Graduate of an approved practical nursing program. Ability to practice independently in the field while appropriately recognizing when RN or physician escalation is required. Strong clinical documentation and communication skills. Reliable transportation and ability to travel to patient homes. Ability to use mobile applications for documentation, EVV, scheduling, and timekeeping. Preferred Qualifications 1+ year of nursing experience. Previous home health, community health, skilled nursing, or field nursing experience. Experience with wound care and chronic disease management. Experience caring for Medicaid/LTC populations. Strong point-of-care documentation skills. Experience managing an independent field schedule.
