About the Role As an LPN on the FACT team, you will play a critical role in delivering high-quality, patient-centered care and supporting individuals on their path to recovery.
Key Responsibilities:
  • Perform accurate withdrawal symptom assessments
  • Ensure compliance with patient requirements (dosing, take-homes, callbacks, and drug screenings)
  • Document services accurately and timely in accordance with policies and best practices
  • Support counseling staff in encouraging patient engagement in treatment
  • Effectively communicate with patients and team members, including via telephone
  • Monitor medical supplies and proactively coordinate replenishment
  • Utilize strong computer skills to retrieve and input patient data Qualifications
  • Graduate of an accredited nursing program meeting Florida Board of Nursing standards
  • Active Florida LPN license required
