Responsibilities Lead and supervise nursing staff within the memory care unit, ensuring high standards of patient care and adherence to all healthcare policies. Develop, implement, and regularly review individualized care plans tailored to each resident's needs, including those with complex conditions such as Alzheimer's or other dementias. Monitor resident health status through comprehensive assessments, vital sign checks, medication administration, and ongoing observation of cognitive and physical changes. Ensure compliance with all state healthcare regulations, HIPAA privacy standards, Medicare guidelines, and other relevant policies governing long-term and memory care facilities. Oversee documentation review and medical records management to maintain accurate, complete, and up-to-date patient files in electronic medical record (EMR) systems. Collaborate with interdisciplinary teams—including physicians, therapists, and family members—to coordinate case management and optimize resident outcomes. Manage shift schedules, staffing levels, and resource allocation to maintain efficient operations while supporting staff well-being and professional growth. Requirements Valid LPN license with current registration in the state of practice. Proven experience in critical care settings such as assisted living, home health, hospice care, or hospital environments—preferably with a focus on memory or behavioral health. Strong knowledge of physiology, anatomy, medical terminology, and nursing practices related to acute and chronic conditions. Demonstrated leadership skills with supervising experience in healthcare settings; ability to manage teams effectively through clear communication and motivation. Familiarity with EMR systems for documentation review and patient monitoring; understanding of medical records management protocols. Comprehensive understanding of health regulation policies including HIPAA, state healthcare regulations, Medicare standards, and quality improvement initiatives.
