Job Summary We are seeking a compassionate and energetic RN/LPN to join our dedicated team. In this vital role, you will deliver exceptional patient care, coordinate medical treatments, and support residents health and well-being in a dynamic senior living environment. Your expertise will help ensure our residents receive the highest quality of care while fostering a safe and nurturing atmosphere. Responsibilities Perform comprehensive patient assessments, vital sign monitoring, and physical examinations to evaluate residents health status. Administer medications and injections accurately following medical orders while maintaining strict adherence to HIPAA and aseptic techniques. Manage complex medical procedures including IV infusions, catheterizations, wound suturing, and ostomy care. Utilize electronic health records (EHR/EHR systems) such as Epic or Cerner for thorough documentation of patient care and medical records management. Collaborate with interdisciplinary teams on discharge planning, case management, and long-term care strategies tailored to individual needs. Provide emergency response during critical incidents such as trauma or acute pain episodes, utilizing critical care protocols including ICU or trauma medicine experience. Support residents with specialized needs such as dementia or Alzheimers care, memory care programs, behavioral health interventions, and hospice & palliative medicine services. Experience Proven experience working in various healthcare settings including hospital medicine, critical care units (ICU/Level I & II trauma centers), emergency rooms, or post-acute care environments. Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; familiarity with ICD coding (ICD-9/ICD-10) is highly desirable. Prior experience in geriatrics, long-term care facilities, skilled nursing homes, assisted living communities, or home & community care settings. Skilled in advanced nursing procedures such as tube feeding management, ventilator support, laboratory specimen collection/processing, and infection control practices. Knowledge of Workers' Compensation law related to occupational health cases is a plus. Experience working with individuals with disabilities or developmental challenges enhances your ability to provide personalized resident-centered care.
