Job Summary We are seeking a dynamic and compassionate Health & Wellness Director to lead and oversee the delivery of exceptional resident care within our assisted living community. This vital role combines clinical expertise with strategic leadership to ensure compliance with regulatory standards, promote resident well-being, and foster a positive, supportive environment for both residents and staff. The ideal candidate will be passionate about improving quality of life, possess strong management skills, and have extensive experience in various healthcare settings including assisted living, nursing homes, home health, hospice care, and acute care. Responsibilities Develop, implement, and monitor comprehensive care plans tailored to residents' individual needs, ensuring alignment with medical and regulatory standards. Supervise nursing staff and care teams, providing leadership, mentorship, and performance evaluations to foster a high-performing team. Oversee clinical operations including documentation review, medical records management, and compliance with HIPAA regulations to safeguard patient information. Ensure adherence to long-term care regulations, Medicare requirements, and other federal/state guidelines through regular audits and staff training. Manage the utilization of Electronic Medical Records (EMR) and Electronic Health Records (EHR) systems for accurate documentation of patient care activities. Collaborate with interdisciplinary teams to coordinate care across home health agencies, hospice providers, memory care units, and acute care services. Lead efforts in quality improvement initiatives focused on patient safety, regulatory compliance, and enhancing resident satisfaction. Requirements Proven experience in assisted living facility environments with a strong understanding of ALF regulations. Demonstrated leadership in managing clinical teams within ALF setting preferred. Knowledge of medical terminology is essential for resident care management. Familiarity with EMR/EHR systems along with proficiency in managing medical records and documentation review processes. Strong understanding of HIPAA compliance standards and regulatory requirements related to patient privacy and safety. Experience with Medicare regulations and managing care plans aligned with federal reimbursement programs. Excellent communication skills coupled with the ability to lead clinical supervision while fostering a compassionate approach to resident care.
