TITLE LPN DESCRIPTION
Full Time or Part-time (including EOW) Shifts are: 7am-3pm , 3pm-11pm, 11pm-7am
Position:

Clinical Coordinator, Oak View Administrator Job Summary:
Primary purpose is to provide routine nursing care in accordance with established policies and prores and current state regulations, as well as supervising personal care assistants, certified nursing assistants and medications technicians.
Core Competencies/Essential Functions and Responsibilities:
Provide nursing care in accordance with physician orders and established plan of care to assure continuity and to meet the individual needs of the resident while assuring resident safety at all times Safe administration of medications and treatments in accordance with recognized standards of practice including infection control. Supervise resident care staff to ensure that facility policies and prores are followed, and that all state and federal regulations are add to. Supervise CNA staff to ensure that facility policies and prores are followed, and that all state and federal regulations are add to. Oversee CNA assignments, and supervise that duties and responsibilities arepleted in a timely and efficient manner. Assist residents/families with concerns or issues they report including management of grievances versus reportable events. Assist with admissions/discharges to/from assigned unit, perform assessments/evaluations as necessary. Observe residents for changes in condition. Notify physician of any changes, incidents, injuries or any other unusual events that require attention and request orders as indicated. Document any change of condition, incidents, injuries or unusual event in the residents' medical record. Take and transcribe physician orders in accordance with facility policies and assist with monthly review of physician order Participate in andplete all required in-servicecational programs andpetency evaluations.
Other Functions and Responsibilities:
Ad to all Oak View Suites and John Knox Village policies and prores. Perform Resident Nurse duties as scled. Assist during disasters as needed. Other duties as requested or assigned.
Working Conditions:
Usually works indoors but may have to work outside during outside events and parties. May have to get into awkward positions or work in cramped spaces. May be exposed to loud noises or sounds that are distracting. May be exposed to diseases or infections from contact with residents or other staff members. Often exposed to contaminants. May sometimes need to wear safety gear such as gloves and or masks. May have to handle hazardous waste in accordance with infections control prores.
Educational, Physical, and Other Requirements:
Must pass Level 2 background screening through the Florida Clearinghouse (Learn more: info.flclearinghouse) Employee must be able to perform the essential functions of the job with or without reasonable amodations. Physical requirements may include frequent use of hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; talk or hear. The employee may be required to lift, stand; walk; sit; climb or balance; stoop; kneel; crouch or crawl; and smell. Vision abilities may require close vision, distant vision, color vision, peripheral vision, depth perception and the ability to adjust focus. For the safety and security of our residents and patients, employees must be able to read, write, and verballymunicate in English, which is the most universally used language of our resident and employee population. May need to climb a 3 foot and sometimes a 6 foot ladder to perform duties such as cleaning and or decorating for holidays and special events. Routinely provides assistance to residents and or other staff members which could require lifting, pushing, pulling or balancing the equivalent of 50 pounds or greater at times. Must be capable of maintaining regular attendance and reporting to work on time. May be required to work nights, weekends or holidays. Needs to be flexible to work other shifts or perform other duties as assigned. Maintain a current active Florida license as a licensed practical nurse. Provide John Knox Village with current license upon renewal of license. Maintain current CPR certification. Must keep all required CEU's up to date for nursing license renewal. Complete and maintain all assisted living specific state required trainings. Maintain annual PPD per state requirements.

Must maintain a professional, respectful, friendly demeanor with all residents, family members, guests, and staff at all times.
