This is a one-time professional engagement for an organization or qualified representative familiar with Florida home health and nurse registry emergency management requirements. Responsibilities Review the applicable CEMP contingency form and requirements. Provide professional guidance regarding the information and documentation needed. Assist with completing the form accurately and appropriately. Sign or certify the form only if authorized to do so and after reviewing the information provided. Identify any corrections or additional documentation required before submission. Requirements Must be a currently licensed Florida home health agency or nurse registry, or an authorized representative of one. Experience with Florida CEMP requirements preferred. Must be able to provide proof of applicable licensure upon request. Must be willing to perform a legitimate review of the form and provide accurate information. Must be comfortable discussing the scope of work and payment terms before beginning. Compensation A one-time payment of $800-$1,000 is available for completed professional services, depending on the scope of work and time required. Payment will be made for actual services performed, not merely for providing a signature. The selected provider must independently determine whether the form is accurate and whether they are authorized to sign it.
