Join our dedicated healthcare team at ProHealth Home Health as a Home Health Licensed Practical Nurse (LPN), where your expertise will directly impact patient well-being and recovery. In this vital role, you will provide compassionate, high-quality nursing care to individuals in their homes, promoting independence and health management. This position offers an exciting opportunity to work in diverse environments, from long-term care facilities to outpatient settings, all while advancing your nursing skills and making a meaningful difference in people's lives. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform personalized care plans. Administer medications, injections, IV infusions, and other treatments following strict protocols and safety standards. Provide wound care, catheterization, tube feeding, and sterile processing procedures with precision and compassion. Document all patient interactions accurately using Wellsky/Kinnser EMR (Electronic Medical Records) ensuring HIPAA compliance at all times. Educate patients and their families on health management strategies, including medication administration, nutrition, and disease-specific care such as Alzheimer's or dementia care. Collaborate with interdisciplinary teams for discharge planning, case management, and coordination of post-acute care services. Skills Prior Home Health experience is preferred. Critical care experience in settings like ICU or Level I Trauma Centers is highly valued. Strong knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Skilled in patient assessment, triage, infection control practices, and emergency response procedures including airway management. Expertise in administering injections, phlebotomy, catheter care, and IV insertion with aseptic technique. Experience working with individuals with disabilities or developmental disorders; knowledge of Alzheimer's/dementia/memory care is preferred. Excellent communication skills for working with diverse patient populations including pediatrics and geriatrics; comfort working in home & community environments. Knowledge of HIPAA regulations to ensure confidentiality and security of patient information; proficiency in medical imaging or laboratory specimen collection is a plus.
