• Job Description:

• Hours:

• M-F 8am
• 5pm; Weekend on-call rotation
• Teams (you will cover areas within your assigned county):
• + Hillsborough County + Pasco County + Pinellas County + Polk County + Sebring/Highlands/Hardee + Other duties as assigned.

+ Develops professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations. + Participates in the review process and the agency performance improvement program. + Provides patient education, medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician. + Displays flexibility in accepting assignment changes and participates in weekend/after-hours coverage and call rotation. + Reports pertinent observations to the attending physician and RN, and implements changes in the patient care plan as directed. + Communicates changes to the appropriate members of the home health care team. + Demonstrates effective communication skills with all team members, patients, and families. + Maintains clinical record information in accordance with established standards and within established time frames. + Provides direct patient care responsibilities for assigned patients in an efficient, organized manner under the direction of the RN. + Performs treatments within their scope of training and administers medications to assigned patients. + Assesses patients during clinical check-in, documents pertinent medical information and vital signs, and assesses for signs of abuse or neglect using age-specific guidelines. + Administers treatments, medications, and follow-up assessments ordered by the provider, verifying as needed, and responds appropriately in clinical emergencies and triage situations. + Performs diagnostic and occupational medicine testing, including EKG, spirometry, audiometry, titmus, and pulse oximetry, and complete forensic testing collections in accordance with established guidelines. + Collects data specific to the age of the patient and contributes to their plan of care. + Receives, verifies, and implements physician orders, ensuring orders taken by the LPN are co-signed by an RN.

• Knowledge, Skills, and Abilities:

• + Demonstrates knowledge of practical nursing theory, modalities, treatments, and skills + A high level of multitasking, organization, and proactive communication skills are necessary + Problem-solving abilities + Positive interpersonal communication skills + Must possess sound judgment and critical thinking skills + Team and customer service oriented + Must be able to manage multiple details, tasks, and decision-making abilities under busy and stressful working conditions + Basic computer skills required including but not limited to MS Office; additional computer knowledge [Preferred] + Venipuncture skills + Must be proficient in the English language; additional languages [Preferred] + Background check meets organizational guidelines + Requires individuals to be able to make emergency and mature independent judgments and decisions + Must have excellent communication skills, be flexible, and able to interact positively with associates + Knowledge with a Point of Care software system for patient care documentation [Preferred] •

Education:

• + High School Grad or Equiv [Required] + Technical/Vocational School graduate of an accredited school of licensed practical nursing [Required]
