Full Job Description Job Overview Responsibilities Provide direct patient care by assisting/administering medications, injections, in accordance with physician orders and established protocols. Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans. Assist with diagnostic procedures such as specimen collection, and order processing. Maintain accurate medical documentation within electronic health record (EHR) systems PCC, ensuring compliance with HIPAA regulations. Support admission planning on weekends, coordinate with management team to facilitate smooth transitions of care. Provide specialized care for patients with complex needs including dementia, Alzheimer's disease. Participate in team-based approaches to patient care involving primary care or hospice services as required.

6a-6p We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings, including long-term care and acute care. As an LPN, you will play a vital role in delivering high-quality nursing services, supporting patient rights, and ensuring compliance with healthcare standards. Your expertise will contribute to creating a safe, respectful, and nurturing environment for our patients and residents. Skills Proven experience as an Licensed Practical Nurse with exposure to Assisted living/long-term care facilities. Strong knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to nursing practice. Experience working with EMR/EHR systems including PCC for efficient documentation and patient management. Ability to perform medication administration via injections (insulin). Familiarity with infection control practices and aseptic techniques essential for maintaining patient safety across all settings. Excellent assessment skills to evaluate patient condition accurately and respond promptly to emergent situations such as airway obstruction or acute pain episodes. Knowledge of specialized areas including geriatrics (senior care) and/or behavioral health is advantageous. Strong communication skills combined with a compassionate approach to working with diverse populations including individuals with disabilities or developmental challenges. Ability to work effectively within multidisciplinary teams while maintaining compliance with healthcare regulations such as HIPAA standards.
