Current Nursing License in the Practicing State; Valid Drivers License; Current CPR Certification; Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting; Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.

Responsibilities:

Administer skilled nursing care to patients in their residence as prescribed by the physician. Assist in planning, coordinating, and implementing care with the RN to ensure quality and cost-effective patient care. Communicate effectively with patients, families, co-workers, and healthcare professionals, documenting outcomes accurately. Support patient and family in learning self-care techniques and assess responses to care. Assist with admitting, discharging, and transferring patients, and perform on-call duties as assigned.
