OhioHealth is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered care including medication administration, basic treatments, patient education, and coordination with RNs and providers. You will document accurately in the EMR, support care plans, and contribute to a safe, inclusive environment for all patients and families. Responsibilities Provide direct patient care under the supervision of an RN and/or provider, including vital signs, specimen collection, and basic assessments within LPN scope of practice. Administer medications and treatments safely, accurately, and in accordance with Ohio Health policies and state regulations. Document all care, observations, and patient responses in the electronic medical record in a timely and accurate manner. Assist physicians and RNs with examinations, procedures, and diagnostic testing, ensuring patient comfort and safety. Monitor patients for changes in condition, promptly report concerns, and assist in implementing appropriate interventions. Educate patients and families on medications, treatments, discharge instructions, and chronic disease management using clear, compassionate communication. Adhere to infection prevention, safety, and regulatory standards, including proper hand hygiene, equipment cleaning, and environment-of-care practices. Participate in team huddles, interdisciplinary rounds, and quality improvement initiatives to enhance patient outcomes and experience. Support care coordination, including preparing patients for discharge, arranging follow up appointments, and collaborating with case management and other disciplines. Required Skills Medication administration and dosage calculation Clinical assessment within LPN scope (vital signs, monitoring, reporting changes) Wound care and basic dressing changes Specimen collection and point of care testing IV therapy and venipuncture (if certified and permitted by scope) Electronic medical record (EMR) documentation, preferably Epic Infection prevention and control practices Patient and family education techniques Basic life support and emergency response support Strong written and verbal communication Interdisciplinary teamwork and collaboration Time management and prioritization in a clinical setting Understanding of HIPAA and patient confidentiality Knowledge of standard precautions and safety protocols
