09/16/2026 Job Description Job Description Full time Part time PRN Opportunities Available Travel Required Across Southern Region Sites Additional Incentive Pay Available Description The Licensed Practical Nurse (LPN) provides high quality patient care through assessment, planning, implementation, and evaluation. This role supports daily clinical operations, assists with basic medical tasks, and collaborates with healthcare and facility leadership to ensure continuity of care. Travel between assigned Southern Region sites (Albany to Columbus) is required. Essential Duties & Responsibilities Deliver high quality patient care and support clinical quality improvement initiatives. Perform diagnostic tests and clinical procedures according to established protocols. Conduct patient assessments, obtain vital signs, administer medications, and provide wound care and emergency first aid. Maintain accurate clinical records, collect data, and prepare required reports. Implement individualized treatment plans and interventions as directed by Health Care Practitioners. Collaborate with multidisciplinary team members to promote health, prevent complications, and support patient education. Attend mandatory training sessions and staff meetings. Assist with basic clinical tasks and support designated facility areas as needed. Ensure compliance with federal, state, and local regulations. Perform additional duties aligned with FCH's goals, values, and operational needs.
