Job Title:

Licensed Practical Nurse (LPN) - CRA Group Home We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide skilled nursing care for a client in an in-home care setting. The ideal candidate must have proven experience working with individuals with intellectual disabilitiesand be confident in providing hands-on care specifically with Insulin Administration and Diabetic Monitoring for individuals.

Responsibilities:

Provide quality nursing care in the client's home setting (CRA Group Home) Perform and assist with 1:1 i nsulin and diabetic monitoring (must have) Assist with proper patient care to ensure comfort, safety, and prevention of complications Monitor the client's condition and report any changes promptly Follow the care plan and physician's orders Maintain accurate documentation of care provided Support the client with daily care needs while maintaining dignity and respect Follow infection control, safety, and agency policies at all times

Qualifications:

Active and valid LPN license Proven experience working w ith individuals with intellectual disabilities Medtech certificate highly required Strong clinical skills and attention to detail Reliable, compassionate, and professional Ability to work independently in an in-home care setting
