Job Overview We are seeking a dedicated and compassionate bilingual( Spanish & English speaking) Private Duty Home Care Nurse-RN/LPN with prior homecare Experience to provide exceptional, personalized nursing care in our client home. As a key member of our caregiving team, you will deliver skilled nursing services tailored to each client's unique needs, ensuring safety, comfort, and improved quality of life. Your expertise will support client with developmental disabilities, and chronic illnesses, fostering independence and dignity in their daily routines. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation to develop individualized care plans. Administer medications and feeding via G/JTubes while adhering to strict safety protocols and documentation standards. Operate medical equipment such as ventilators, catheters, and oxygen therapy devices while maintaining infection control standards. Document all patient interactions accurately using electronic health records (EHR/EHR systems), ensuring compliance with HIPAA and long-term care regulations. Provide patient education on health maintenance, medication administration, nutrition, and safety measures tailored to individual needs. Collaborate with physicians, case managers, and caregivers to coordinate discharge planning, home modifications, and ongoing care strategies for optimal outcomes. Skills Proven experience working as an LPN in diverse settings such as home care, skilled nursing facilities, hospitals (including ICU and trauma centers), or hospice environments. Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and clinical procedures including venipuncture and sterile techniques. Skilled in managing acute pain cases, infectious disease protocols, IV infusion therapy, and respiratory support including ventilator management. Familiarity with EMR/EHR systems like Epic, Cerner or Athenahealth for accurate documentation and patient record management. Excellent patient assessment skills with the ability to recognize changes in condition promptly; experience triaging urgent situations effectively. Ability to communicate clearly with patients of all ages—including pediatrics—and collaborate seamlessly with multidisciplinary teams. Knowledge of long-term care regulations and experience working with individuals with developmental disabilities or cognitive impairments such as Alzheimer's or dementia care.
