Licensed Practical Nurse (LPN) Job Summary Gateway Healthcare Agency is seeking a dependable, compassionate, and professional Licensed Practical Nurse (LPN) to join our growing healthcare team. The LPN will assist with providing and coordinating quality care to individuals receiving services in their homes and community settings. This position includes client visits, health monitoring, documentation, caregiver support, medication-related oversight within the LPN's scope of practice, and communication with the supervising Registered Nurse and agency management. We are seeking an LPN who is organized, dependable, comfortable working independently in the community, and committed to providing excellent client care. Key Responsibilities The Licensed Practical Nurse will perform duties within the LPN's authorized scope of practice and under appropriate RN/physician direction, including: Conduct scheduled nursing and client visits as assigned. Obtain and document vital signs and other relevant health information. Observe clients for changes in physical, behavioral, or functional condition. Promptly report significant changes or concerns to the supervising RN and appropriate agency personnel. Follow established plans of care and nursing instructions. Assist with medication monitoring and medication-related services within the authorized scope of practice. Review medication information and report identified discrepancies or concerns. Provide client and caregiver education as directed by the supervising RN. Assist with monitoring caregiver compliance with established care plans. Observe the quality and safety of services being provided in the client's home. Report caregiver performance, attendance, documentation, or client-care concerns to appropriate management. Complete accurate and timely nursing documentation for all assigned visits. Assist with follow-up after hospitalizations, emergency room visits, incidents, or changes in client condition as assigned. Maintain professional communication with clients, families, caregivers, nurses, case managers, and agency management. Maintain client confidentiality and comply with HIPAA requirements. Participate in staff education, competency reviews, and quality-improvement activities as assigned. Travel to client residences throughout the agency's service area. Required Qualifications Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Georgia Graduate of an approved practical nursing program Current CPR/BLS certification Valid driver's license Reliable transportation Current automobile insurance Ability and willingness to travel to clients' homes Strong communication and documentation skills Ability to work independently and follow established clinical direction Dependable attendance and time-management skills Ability to use electronic documentation systems and basic computer applications Preferred Qualifications Previous home health, private home care, community-based nursing, long-term care, or similar nursing experience Experience working with elderly individuals and individuals with disabilities Experience with Georgia Medicaid waiver services Familiarity with CCSP, SOURCE, and Structured Family Caregiving (SFC) Experience with medication monitoring and caregiver education Experience completing home visits and nursing documentation Ideal Candidate The ideal LPN for Gateway Healthcare Agency is someone who is dependable, organized, compassionate, observant, and accountable . Because much of this position involves independent work in clients' homes, the LPN must be able to manage scheduled visits, communicate effectively with the supervising RN and management, recognize and report changes in client condition, and complete all required documentation accurately and on time. Compensation Competitive compensation based on experience and assigned responsibilities. Compensation may be structured as hourly or per-visit depending on the position and assigned caseload. Mileage and travel arrangements will be discussed during the interview process.
