Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for pain control, hospice care, or chronic conditions. Assist with diagnostic evaluations and procedures such as catheterization, specimen collection/processing, and wound care using aseptic techniques. Manage airway and ventilator support for patients requiring respiratory assistance; provide emergency first aid and participate in airway management during acute episodes. Document all patient interactions thoroughly in medical records while maintaining strict adherence to HIPAA regulations and clinical confidentiality policies. Collaborate with interdisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure seamless continuity of care for patients with complex needs like dementia or developmental disabilities. Provide education to patients and families on health coaching, medication administration, nutrition, and long-term care regulations to promote informed decision-making. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice. Proven experience working with diverse populations including geriatrics, pediatrics, neonatal or adult patients in settings such as long-term care facilities, hospitals (including ICU or trauma centers), outpatient clinics, or home care environments. Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and clinical procedures such as venipuncture, tube feeding, catheter care, and basic life support (BLS). Experience with EMR/EHR systems like Epic, Cerner or Athenahealth; familiarity with medical imaging documentation is a plus. Critical care experience (ICU or Level I/II trauma centers), emergency medicine exposure, or hospice experience highly preferred. Ability to perform physical examinations accurately and conduct diagnostic evaluations efficiently while demonstrating excellent caregiver communication skills. Knowledge of infection control protocols and long-term care regulations; experience working with individuals with disabilities or dementia is advantageous.
