Job Summary The Full-Time Hospice Licensed Practical Nurse (LPN) provides compassionate, high-quality end-of-life care to patients in private homes, assisted living facilities, nursing homes, and inpatient settings. Working under the supervision of an RN and in collaboration with the interdisciplinary hospice team, the LPN provides symptom management, comfort-focused care, patient and caregiver education, and nursing services in accordance with the established plan of care. Key Responsibilities Provide skilled nursing care in accordance with the hospice plan of care and within the LPN scope of practice. Perform focused assessments and monitor for changes in pain, breathing, cognition, skin integrity, nutrition, elimination, and overall condition. Administer medications, treatments, and comfort measures as ordered. Report significant changes, uncontrolled symptoms, safety concerns, and medication effectiveness to the RN Case Manager and/or hospice provider. Educate patients and caregivers on medications, symptom management, disease progression, safety, and end-of-life care. Provide emotional support and compassionate guidance to patients and families. Collaborate with RNs, physicians, nurse practitioners, aides, social workers, chaplains, and other members of the interdisciplinary team. Assist with coordination of medications, supplies, durable medical equipment, and other patient needs. Maintain accurate, timely documentation in the electronic medical record. Participate in interdisciplinary team meetings, required education, quality improvement activities, and on-call responsibilities as assigned. Qualifications Graduate of an accredited Practical Nursing program. Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Georgia. Valid driver's license and reliable transportation. CPR/BLS certification preferred or as required by agency policy. At least one year of nursing experience preferred. Hospice, palliative care, home health, assisted living, long-term care, or related experience preferred. Strong communication, organization, time-management, and clinical observation skills. Ability to work independently in the field while maintaining close communication with the care team. Compassionate, dependable, professional, and committed to patient comfort and dignity.

Working Conditions Schedule:

Full-time, typically Monday-Friday (9:00-5:00).

Travel:

Regular local travel to patient homes and care facilities throughout the greater Atlanta area.

Physical Requirements:

Ability to perform routine nursing duties, including standing, walking, bending, repositioning patients, and assisting with transfers as appropriate.
